Why injections eventually stop working

Cortisone temporarily suppresses inflammation chemically — and that's the strength, but also the weakness. The underlying inflammatory cells continue producing pain-generating cytokines once the cortisone clears. Each subsequent injection often provides less relief and shorter duration. Gel injections add cushioning but do not address inflammation. PRP delivers growth factors but cannot interrupt the chronic inflammatory cycle in advanced osteoarthritis.
