What Is Ledderhose Disease?
Ledderhose disease, also called plantar fibromatosis, is a condition in which firm, fibrous nodules develop along the plantar fascia on the bottom of the foot. These nodules are benign but can grow over time, causing significant pain when walking or standing. In some cases, the nodules form cord-like bands that pull on the toes, further limiting mobility.
The condition is related to Dupuytren's contracture, which affects the hand, and shares a similar pattern of excess collagen and fibroblast activity. It is more common in men, people of Northern European descent, and individuals with a family history of fibromatosis conditions.
Treatment options for Ledderhose disease are limited. Orthotics and padding can redistribute pressure away from the nodules but do not stop their growth. Cortisone injections may temporarily reduce pain but do not shrink the nodules. Surgical excision carries a high recurrence rate, sometimes exceeding 50 percent, and can result in prolonged recovery and complications including nerve damage and chronic wound-healing issues.
How LDRT Treats Ledderhose Disease
LDRT is considered one of the most effective treatments for early and moderate-stage Ledderhose disease. European studies have shown that low-dose radiation therapy can slow or halt nodule growth, reduce pain, and in some cases soften existing nodules.
The treatment works by targeting the actively proliferating fibroblasts within the nodules. Low-energy radiation interrupts the overactive cellular process that produces excess collagen, effectively putting the brakes on disease progression. When administered during the earlier stages of the condition, LDRT can prevent the need for surgery altogether.
Each session is brief, painless, and entirely external. There are no injections into the sole of the foot and no recovery time.
Who Is a Candidate for LDRT?
LDRT for Ledderhose disease is most effective in patients with early to moderate disease, but it can also benefit patients with more advanced conditions. You are a strong candidate if:
- You have been diagnosed with Ledderhose disease and your nodules are growing or becoming more painful
- Orthotics and padding have not adequately managed your symptoms
- Cortisone injections have not slowed nodule growth
- You want to avoid surgical excision and its high recurrence rate
- You also have Dupuytren's contracture and want a treatment that addresses both conditions
Heelex Surprise is one of the few clinics in Arizona offering LDRT for fibromatosis conditions. For Ledderhose disease specifically, LDRT has a stronger evidence base than nearly any other non-surgical option.
What to Expect
Your initial visit includes a clinical examination of your foot, a review of imaging and biopsy results if available, and a detailed discussion of your treatment history. If LDRT is appropriate, you will typically undergo six to ten sessions spread over several weeks.
Sessions are painless and take only a few minutes. You can walk normally between treatments, and there is no downtime. Patients often notice that their nodules stop growing and pain diminishes in the weeks and months following treatment. Some patients also experience softening of existing nodules over time.
